Once a patient is discharged, they can have up to 8 medications to take. Side effects, dosages and simply remembering to take them are common problems once patients are home. SMH provides a very detailed discharge document, a hotline for questions, and caregiver support.
Supportive care services at SMH are born from the essence of palliative care; easing difficult conversations, family focused care and quality of life. Providing means for openness can help at a critical time in one’s life.
Patient Care Services is a patient centered program that helps the individual person in their health care journey. The dedicated team works closely with The Breast Heath Grant Support Program, providing information and help.
